Annual Awards Nomination Form
Each Region may submit ONE nominee per category, per year. Nominations should be submitted by the Commanding Officer, Executive Officer (First Officer) or an authorized representative (Must be approved in advance by the Regions Awards Director). Nominations MUST be submitted on this form or they will not be accepted. Nominations for the Annual Awards will be accepted from January 1 through January 31.
NO NOMINATIONS WILL BE ACCEPTED AFTER JANUARY 31ST.
